Encompass delivers a wide range of services including adult social care and homelessness support.

For this job role, you will be working in the Brokerage Team to arrange care for customers in their own homes and in residential care at the request of Social Workers employed by Sutton Council.

In this job role you will:

  • Work collaboratively with Social Workers, START Team (Hospital Discharge), Safeguarding Team to ensure a smooth transition
  • Manage the throughput of work via a duty system that includes telephone enquiries, emails and direct referrals
  • Assess customer needs and identify potential support barriers and solutions
  • Find and facilitate access to accommodation and support as appropriate
  • Ensure appropriate person-centred arrangements match support plans
  • We are looking for someone who has:

  • Knowledge of the 2014 Care Act, 2005 Mental Capacity Act and DoLS
  • Significant experience of the Care Sector
  • Demonstrable customer-focused attitude with a desire to resolve issues and find solutions
  • The ability to build positive relationships, working collaboratively with others
  • The ability to communicate effectively
