UCLH has a permanent and exciting vacancy within the Integrated Discharge Service.

The Integrated Discharge Service (IDS) team is recruiting for a Band 5 Safe Discharge Coordinator to support the Integration agenda at UCLH. The team are looking for a highly motivated and enthusiastic coordinator who will be able to support and coordinate discharge in a number of different specialties and locations to improve patient care and the patient experience within the Trust. This posts will play a pivotal role in supporting the smooth transfer of care from UCLH to the community services and offer an exciting opportunity for someone with previous healthcare experience to build upon their knowledge and experience in this innovative position.

Main duties of the job

You will work under the supervision of the registered nursing team and/or clinical leads, but will work independently in facilitating a smooth and timely discharge process for patients. This will include supporting the patient discharges from wards in order to create new capacity, but ensuring that the continuity of care is maintained. Participating in the discharge process will mean assisting in patient care, supporting the Integrated Discharge team (Complex Discharge, Admissions Avoidance, homeless team, alcohol and drugs team,) where necessary. The role also involves undertaking both routine and specifically identified tasks for which the post holder will be trained and assessed as competent.

In order to be successful in the role, the applicants must have a keen interest in improving the patient experience of discharge planning and patient flow on the ward, and care about the continuity of the service between the acute and community setting. You will have good knowledge and experience in the acute setting, along with a NVQ level 3 or equivalent. Further training will be made available in the specialty.
