An exciting opportunity has arisen for a dynamic and enthusiastic Band 7 Specialist Clinical Lead Nurse for Homeless Teams to join our Homeless Community Mental Health Team, Rough Sleeper Mental Health Outreach Team, and Primary Care Mental Health Team at the Homeless Health Exchange.
Our teams operate a city-wide service to meet the complex needs of homeless adults with both primary care mental health difficulties often as a result of trauma and severe and enduring mental health problems, for many individuals this is associated with comorbid substance misuse issues and who require significant engagement due to their chaotic and transient lifestyle.
The post holder will be responsible for the coordination, planning and implementation of evidence-based care to service users, provide professional advice, support and leadership to clinical staff across the three teams and monitor quality standards relating to service user and carer experience, and key performance indicators, ensuring data quality is maintained.
We require someone who wants to be a team player and is flexible enough to meet the challenges of the post which involves working on outreach, in hostels, primary care and with partner agencies.
We can offer you a wealth of experience in delivering care to this vulnerable group working in partnership with other statutory and third sector agencies. We have good team support and great opportunities for personal development and on-going training.
Main duties of the job
The post will involve being responsible as the Homeless Teams Lead Nurse for the assessment and coordination of care needs, planning and implementation of evidence-based care to homeless adults in a city wide service within the Homeless Community Mental Health Team, Mental Health Rough Sleepers Outreach Team and The Homeless Primary Care Team at the Health Exchange.
The post involves working alongside the team manager to ensure quality and standards within the team are set, monitored, maintained and evaluated. The post holder must develop a close working relationship with the multi-disciplinary team, supervise staff and oversee the team’s day to day processes to ensure they deliver its performance, trust targets as well as ensuring positive interfaces with partner services and stakeholders. You will require excellent communication skills and be expected to carry a small caseload.
