37.5 hours per week. This is an exciting opportunity to provide anew model of care across Plymouth, through the establishment of the HealthInclusion Pathway-Plymouth.

In line with currentbest practice this service is being developed using the Pathway approach, providingend to end support for individuals who are homeless. It involves not onlymedical staff, but a range of multidisciplinary professionals with expertise insocial care, housing law and benefits issues, ensuring that a persons fullrange of needs are supported.

This service will be delivered through an exciting and innovativepartnership approach with collaboration from organisations such as LivewellSouthwest CIC, Adelaide and St Levan Surgery, Pathway UK, University HospitalsPlymouth NHS Trust and the Plymouth Alliance.

    Promotethe image of the department, checking that notices and leaflets are up to date& well presented.

    Ensure that allcomplaints, compliments, concerns, comments and suggestions are appropriatelyrecorded, and where possible resolved within level of role or escalatedappropriately, supporting junior staff where necessary.

    Deal with allreferrals/e-referrals directed to the team e.g. Refer to MDT, upload documents,accept/decline referrals, request any additional information from referrersetc.
