Job summary
We are looking to recruit a dynamic nurse to work within our High Intensity User (HIU) Team in Walsall. The post holder will support the HIU lead working internally and externally in delivery of the project aims, initiatives and services and in a cost-effective way, adjusting plans as appropriate.
This is an opportunity for a Registered RN or RMN to work directly with a cohort of patients who frequently attend emergency and unplanned care services, to reduce the number of times they attend through engagement and collaboration.
The post is advertised as a 6-month fixed term, with the possibility of a permanent position subject to review and ongoing success of the project.
Main duties of the job
To case manage a cohort of patients who frequently attend emergency and unplanned care services. Typical presentations are those that are homeless, may self-harm, or have mental health or substance misuse problems.
The post holder will work closely with key professionals and stakeholders within Primary, Social and Community Care to develop bespoke care plans for individuals.
The post holder will attend MDT meetings and Case Conferences in a variety of settings and liaise with external agencies including the voluntary sector.
