Base HAST Team Office, Hanover Medical Centre, 100 William St, S10 2EB Liaises with : Changing Futures Core Team, Housing solutions including the Sheffield Rough Sleeper Initiative and Temporary Housing accommodation services. Voluntary organisations supporting the homeless and those involved with domestic violence, substance misuse and the criminal justice service including probation, police and liaison and diversion services. Other SHSC Trust specialist services, Community OTs, Primary Care and other Secondary Mental Health services SPA and Community Recovery Mental Health Services.

Main duties of the job

The Changing Futures programme is a £64 million joint initiative by the Department for Levelling Up, Housing and Communities (DLUHC) and The National Lottery Community Fund, the largest funder of community activity in the UK. The fund is for local organisations to work in partnership to better support those who experience multiple disadvantages, including homelessness, substance misuse, mental health issues, domestic abuse, and contact with the criminal justice system.

Sheffield is one of fifteen areas nationally selected to deliver an ambitious £3.267 million programme between now and March 2024 and we want passionate, skilled, and driven staff to turn our plans into reality.

HAST is one of the services chosen for expansion and includes the addition of this post. Youwill carry an OT caseload and provide clinical interventions on a 1:1 or group basis. You will engage service users through an assertive outreach model or at specified locations on NHS premises and/or locations on the homeless pathway.

You will work in a trauma informed way and encourage learning or relearning of skills in activities of daily living, coping skills, social skills and support their development of healthy roles.

The successful candidate will be supported with their professional development through professional, management and clinical supervision as well as gaining access to a national network of occupational therapists working in homelessness.

Principal Clinical Duties

  • To select and apply Occupational Therapy Assessments for a designated caseload, addressing occupational performance and skill deficits, enabling the client in areas of self-maintenance, productivity, and leisure.
  • To undertake generic initial assessments and collaboratively identify with the client priorities for therapy
  • To be professionally and legally responsible and accountable for all aspects of work.
  • To undertake the OT assessment of clients using investigative and analytical skills to formulate individual treatment plans
  • To accept clinical responsibility under supervision for a designated case load some with complex presentations prioritising need and taking on the role of care co-ordinator where relevant .
  • To plan and implement client-centred individual and group interventions, using clinical reasoning and a range of treatment interventions to achieve therapeutic goals.
  • To monitor, evaluate and modify treatment for clients to measure progress and ensure effectiveness of intervention
  • To demonstrate an understanding of the effect of disability and recommend adaptations to the clients social and physical environment.
