An exciting opportunity has arisen for a dynamic, enthusiastic and motivated individual to join the ICP as a Band 6 Integrated Discharge Sister. The role of the Discharge Sister will be responsible for facilitating all aspects of the discharge process, including complex discharges, liaising with families, external agencies and the multi-disciplinary teams, ensuring all discharges are completed in a timely fashion to ensure flow through the hospital.They will also be responsible for managing their own cases load of patients ensuring all aspects of the discharge have been addressed. Training will be provided and you will be supported by current post holders.

Main duties of the job

To work as an autonomous practitioner to perform a specialist nursing assessment patients. To use clinical reasoning to deliver and adapt individualised discharge plans, involving patients and carers/relatives in the planning and delivery.

To be professionally responsible for managing a defined caseload of complex cases and lead Discharge Planning/Best Interest Meetings and Professionals meetings as required.

To ensure the Trust meet its statutory responsibilities to the homeless patients by ensuring a full assessment has been undertaken.

To facilitate Red and Green Multidisciplinary Meetings daily on the wards to ensure active discharge planning is being undertaken to progress the patient to their date of discharge.

To identify which discharge pathway the patient requires and in conjunction with the ward staff ensure this is documented on tele tracking at the daily meetings.

Those individuals who have been identified on Pathway 4 as requiring further period of rehabilitation the post holder is to ensure that the individual meets the criteria and the intermediate care checklist has been completed to ensure a safe and timely transfer of care.

To provide education/training to the ward staff and other members of the multidisciplinary team in all matters related to discharge planning.
