Job summary

An exciting opportunity has arisen for a motivated Band 5 registered nurse to work within the CCN service, clinically. The opportunity will enable the successful post holder to develop a variety of skills to support a career change into community practice. The post holder will have the opportunity to develop their clinical skills and sphere of practice, working closely with the Band 6 clinical / care closer to home lead and other members of the CCNS.

The successful candidates will be required to work day shifts across the working week within the clinical / care closer to home arm of the service. The training opportunities and additional training will be supported, and the post holder will be mentored to develop their skill set, reflective practice and autonomy supporting them when working within the community.

To work as a member of the CCNs and the wider multi-disciplinary team to ensure that the child/ young person receive a high standard of nursing care by adopting a family centred approach, working as part of the multi-disciplinary team, assessing, planning, implementing and evaluating care to ensure that the needs of the child/ young person are met.

Main duties of the job

  • To work as band 5 nurse providing nursing care to children/ young people within the community.
  • To participate in the discharge planning of a child/ young person with healthcare needs from hospital to the community.
  • Provide evidence based care whilst maintaining a good working relationships with colleagues within acute and community settings to ensure collaborative working and good communication.
  • Take an active role in maintaining effective communication with all members of the multi-disciplinary team. Ensures that communication with the children and their families is effective in meeting the sensitive needs of the family.
  • To work collaboratively and communicate effectively with a range of professionals, acting in the best interests of the child/ young person at all times.
  • To complete comprehensive and accurate documentation, assessments and reports
  • Work within designated levels of competence to ensure safe practice seeking support and guidance where required to ensure the delivery of safe and appropriate care to the child/ young person.
  • To support the work of the specialist nurses.
  • To teach and assist the carer to give ongoing nursing care, and support them in this role.
  • To be conversant in the functions of equipment required and report faulty equipment.
  • To adhere to the All Wales and Health Boards Child Protection Procedures.
