UCLH is seeking to appoint a consultant in Acute & General Medicine with a special interest in Inclusion Health. This will be a 10 PA post commencing in early 2023.

The acute medicine department is a lively and enjoyable place to work and thrives on a having a close knit team. Clinical work takes place across 3 areas; acute medical take, the acute medical unit (AMU) and the ambulatory care clinic. Our SDEC unit comprises both acute take and ambulatory Care. The successful applicants will undertake weekend cover for Acute Medicine and join the senior physician on call rota.

A key challenge within Inclusion Health (IH) is providing integrated health and social care to socially complex patients who have high levels of physical and mental health multi-morbidity complicated by problems such as addiction, homelessness, contact with criminal justice system, and migration-related concerns. The healthcare of such groups is complicated by high levels of stigma, chaotic lifestyles, frequent use of emergency services and appalling health outcomes. IH aims to prevent and address the harms of extreme inequity through clinically focussed service models, research, teaching and advocacy.

Main duties of the job

This post has been created to bring new clinical leadership to innovative developments in IH at UCLH.

With the creation of new structures within the ICS for community support of homeless and other vulnerable groups, alongside evolution in the way these highly vulnerable inpatients are identified and managed within the Trust, comes an opportunity to define clinical leadership for IH at UCLH. This is invested in a 0.5WTE Consultant in IH embedded within an acute medical post but delivering liaison care to other inpatient wards, supported by the drug and alcohol, mental health, members of the integrated discharge team, and linked with Pathway, Find & Treat, Camden Adult Pathway Partnership (CAPP), Homeless Intermediate Care Team (HICT) and other community provision.

Key areas within UCLH managing this cohort of patients are Emergency Services (ED and AMU), and T8 (inpatient infectious diseases); however, there are other services such as Trauma and Orthopaedics (for complex skin, soft tissue, and bone infections), other Surgical, Women’s Health and Medical Specialties are also relevant touchpoints for these patients.

The role will also support the nursing teams managing patients after discharge to the community.

About us

Department of Acute Medicine

In recent years the Acute Medical Unit (AMU) has undergone a major reconfiguration in service for acute medical admissions with increased consultant input within the initial 24-48 hours following admission, providing a consultant-led service for 12-hours a day, 7-days a week.

The AMU occupies 56 beds on the first floor of University College Hospital; it has close working relationships with the Emergency Department, Intensive Care Unit and Medical Specialties and Infection Divisions and several AMU consultants have joint contracts with one of these areas. AMU incorporates an Enhanced Care Unit with 18 monitored beds. The department is well supported by specialist teams with daily consultant ward rounds in cardiology. There is a resident therapies team and several new initiatives for discharge support.

For Acute Medicine, the Same Day Emergency Care area comprises the acute medical take and Ambulatory Care Unit and is located next door to the Emergency Department. Staffing includes 2 Consultants (1 take, 1 Amb Care) and a Nurse Consultant in Acute Care. All referrals to ambulatory care are received by the consultant and are triaged to admission or ambulatory care. An in-reaching model into the Emergency Department is operated to maximize admission avoidance processes.
