We have a fantastic new job opportunity for a full time DigitalTransformation Lead within Trafford West Primary Care Network. You will becomea valued member of our primary care team, focused on providing proactive,personalised, co-ordinated and integrated health and social care to theTrafford West community.

Main duties of the job

As Digital and Transformation lead you would be expected to support increased access to care for patients, by supporting the adoption and/or optimisation of new technology and other initiatives to improve the care offer and enabling PCN staff to work more effectively to support the sustainability of general practice services. This is likely to be an evolving role that embraces digital technology to enhance care, support improving population health and embed operational efficiency within primary care.

The role will involve the development of Managed Clinical Networks and will involve collaboration with a broad range of stakeholders, in order to reduce health inequalities and improve health within the population of Trafford West.

Interrogate and analyse, sometimes highly complex, information to support the delivery of clear project reporting and project outcomes. Design, run and analyse

Write and submit regular high-level project reports for the programme board showing progress against milestones, status, resource requirements, issues, risks and dependencies, making recommendations where a range of options may exist.

Develop key performance indicators and milestones for specific programmes and projects, managing conflicts and ensuring that interdependencies are managed effectively.

Use data, and improve data quality, to understand demand, capacity, activity and hit government set targets.

Analyse changes (including new services) to core contracts, enhanced services (DES), Impact and Investment Funds (IFF), Primary Care Standards (PCS) and Quality and Outcomes Framework (QOF) indicators.

Document recall, monitoring, and prevention systems for each long-term condition, and use this to prompt periodic review to ensure we are following good clinical practice and maintain safe, efficient systems. In particular, community planning staff, Health and Social Care staff, Third Sector and other relevant organisations, by providing support, expertise, learning and development opportunities.

Identify, analyse, interpret, and present research, data and undertake local needs assessments and audits to support the development of a Public Health evidence base to influence policy and programme development and build sustainability, providing written reports on work progress for key stakeholders and partners.
