This is an exciting opportunity for compassionate nurses to join a team that provides health care to the homeless population of Central London and to act as an advocate, liaising with both statutory and voluntary agencies to ensure best outcomes for patients.

This is for a fixed term, initially 6 month.

You will use a case management approach supporting people experiencing multiple disadvantage, unable to engage with existing specialist services. They will have experienced adverse childhood events compounded in adulthood through their homeless experience. They may not have the skillset set to self care or engage with health services. Consequently, they have significant unmet physical health problems including severe and enduring mental health problems, chaotic drug use and change resistant alcohol use.

Main duties of the job

Case management will require a whole person trauma informed approach with the nurse following the patient on their journey. This will involve in reach to patients placed in temporary accommodation, hostels and rough sleeping on the street many of whom will move within the pathway. The role requires a trauma informed approach delivered at a pace suitable for the patient and to develop interests and confidence engaging with health. The nurse will be required to deliver advanced nursing care as well as plan and coordinate multidisciplinary working to achieve this outcome.

We are looking for non-judgemental, skilled and dynamic nurses with a commitment to providing high standards of care for this vulnerable group. You will need to be flexible, an excellent problem solver, and have the ability to work as a lone nurse during clinic time.

Candidates should have excellent communication skills and a background in primary care, preferably as a practice/walk-in centre nurse. The job is Monday to Friday 9am to 5pm
