Main duties of the job

The Specialist Rotational Pharmacists at UHSx provide clinical pharmacy services to a range of areas such as paediatrics, surgery, dispensary, emergency medicine, critical care, care of the elderly and specialist medicine.

All pharmacists on the programme have the chance to contribute to the progression and development of clinical pharmacy services, undertake clinical audit, line management, and be involved in teaching and training. The rotations are designed to enable hard-working pharmacists develop their skills in these and other areas, and to facilitate good practice and innovation.

The successful candidate will work as a part of a broad team of pharmacists and other healthcare professionals, ensuring that patients remain our central focus. On our wards, fully trained medicines management technicians assist with medicines reconciliation and we have clinical assistants which undertake patient’s own drug (POD) checking and medication ordering. This allows for our pharmacists to focus on clinical aspects of care. We have established electronic prescribing and medicines administration (EPMA) to improve patient safety and streamline the patient journey.

The successful candidate will need to have hospital pharmacy experience, preferably in an acute hospital setting, excellent communication skills and the ability to demonstrate clinical experience in a broad range of specialisms.

  • To build and maintain excellent communication and working relationships with all members of the pharmacy team and wider multi-disciplinary team.
  • To ensure that medicines management issues are effectively communicated with all members of pharmacy staff, patients, their relatives and carers and other health professionals and staff to reduce the risk of problems arising when patients transfer between wards and organisations.
  1. Analytical and Judgemental skills – (Factor 3)
  • To provide a specialist clinical pharmacy service on a rotational basis to defined areas of the Trust. The role will also include participation on daily consultant ward rounds for the provision of treatment related recommendations and safety interventions.
  • To assist the Lead pharmacists in monitoring the effectiveness of services and in the development of services.
  • To participate in the provision of clinical pharmacy services to general and specialist wards.
  • To ensure that medicines reconciliation is accurate and recorded, that initial prescription charts are accurate for new patients.
  • To respond to requests for medication supply or advice in the on-call setting and to ensure safe and effective medication availability for patients of the organisation; that medical staff and nursing staff have the knowledge and support needed.
  • To make appropriate decisions and judgements in relation to requests.
  • To be aware of risk management issues with respect to medicines use and to actively seek to reduce the risk of medication errors and incidents on your ward areas and across the trust.
  1. Planning and Organisational skills – (Factor 4)
  • Assist in the further development of the Specialist rotational pharmacist programme by providing honest and constructive feedback.
  • To assist the relevant pharmacy team, according to rotation, by prioritising own work and that of the service to ensure maximum benefit to the patient, clinical area and organisation.
