This is an exciting opportunity to work within a new, enthusiastic and innovative Rough Sleepers team within West London NHS Trust with excellent reviews from all stakeholders and has a wide demographic profile covering all areas of Hounslow, Ealing and Hammersmith & Fulham.

The team provides assertive community mental and physical health care within a multidisciplinary team context for rough sleepers and promotes collaborative and responsive care towards re-engaging the rough sleepers with their local communities over a period of time, working alongside our community partners. The outreach sessions are on a shift basis (early morning or late evenings) and is managed on a rota basis with other RAMHP team members. The appointee will play an integral role in working alongside the MDT in all aspects of its care through providing assessments, brief intervention, follow up, assertive outreach, and joint outreach sessions with St Mungo’s or Thames Reach. You will be working alongside and under supervision of the Consultant Psychiatrist in further developing the service, teaching and training new team members and primary care staff and providing clinical and managerial leadership to the team. It is expected that as the service is expanding with renewed funding in place the remit of the service, could include an in-reach aspect into accommodation for the rough sleeping population in the boroughs.

The post will cover up to 5 sessions per week (negotiable).

Main duties of the job

You will have a key role in assessment, outreach, liaison and consultation model to provide early, easy access and advice to all community partners to support care of rough sleepers in the locality. You will also provide clinical leadership to the team working alongside Dr Jose Maret, Consultant Psychiatrist, in clinical multi-disciplinary team meetings and referrals meetings as well as being available for case discussions, advice and supervision within and outside team meetings.

You will also have a role in the management of a smaller number of more complex cases, to offer senior assessments and more direct input in particular in risk and/or medication management and diagnostic assessments.

This will also include Mental Health Act Assessments, CTO reviews and supporting the duty function of the team with urgent concerns and assessments. You will have senior medical responsibility under supervision of the Consultant Psychiatrist for the care carried out by the team.

The Rough Sleepers Team is a specialist mental health service providing community mental health care via outreach for street homeless people covered in the three Boroughs of Ealing, Hammersmith and Hounslow.

The Team is part of the Community and Recovery Service (CARMHS) pathway in West London and part of a wider network of statutory and 3rd sector organisation supporting homeless healthcare across the three Boroughs. The team was set up as a London Mayor and Greater London Authority (GLA) initiative in 2020 to provide essential care for vulnerable street homeless people in the London area.

Our principal partner in achieving this essential work is the Homelessness Charities, St Mungo’s Outreach service and Thames Reach, which operates in varying arrangements in all three Boroughs.

This will include developing strategy, quality improvement, audit, and written policies and guidelines for the future delivery of RAMHP services through working collaboratively with all stakeholders.

You will have the opportunity to participate in training alongside the Consultant Psychiatrist and Team Manager for key services and stakeholders with the WLT and in the community. This may include formal didactic teaching; individual clinical teaching based around individual cases; developing strategies for teaching and training.

You are also likely to contribute to complex case forums across disciplines, locality meetings and other community fora to improve clinical care and communication across services.

There is opportunity to provide teaching to Imperial College Medical Student placements on rotation within the post.
