Job summary

The REACH Team is a multi-agency complex care team working with those who are street homeless and have experienced multi-disadvantage. As the Clinical Nurse Specialist for the Team, you will work as part of a unique housing first project to identify and support those entrenched in a cycle of rough sleeping, sofa surfing or are at risk of becoming homeless. We, as a multi-agency team, support the people who use our service to manage their mental health, substance misuse needs and work to reduce the harms they experience to themselves or others. Onward referrals will also be made to agencies that can provide assessment and support to meet an individual’s needs that are not within the scope of the team.

Main duties of the job

Assessment of Mental Health needs, offering a holistic formulation of need.

Direct clinical intervention role, (building engagement, motivational interviewing, harm reduction) supporting movement towards engagement in mainstream service offers (Mental health, housing, drug and alcohol, physical health).

Supporting services to hold a holistic plan of care, which holds the wider service users’ needs in mind

Risk assessment

Physical health advice to, persons in receipt of support services and others.

Person Specification

Qualifications

Essential

  • Current Professional Registration with Nursing and Midwifery Council – RMN / RNLD
  • Recognised Practice Assessor/Practice Supervisor or currently in progress of achieving.
  • Trained in Clinical Supervision and providing Clinical Supervision or in the process of achieving
  • Postgraduate diploma in a subject relevant to the area Or be willing to work towards and achieve within 2 years and
  • Within the first year of being in post hold a recognised qualification in a governed therapy/advanced clinical qualification Intermediate Systemic Family Therapy/Post Grad diploma in CBT/British Isles Dialectical Behaviour Therapy /NMP (Independent Prescriber)
  • And have Equivalent clinical experience to that of a Post Grad Diploma/master’s level such as working as a band 6 (or above) in relevant clinical area with significant experience (see below under experience), together with evidence of relevant CPD
  • Knowledge

    Essential

  • Extensive knowledge of current issues relating to health needs of patients in the field of practice, regionally and nationally
  • Understanding and working knowledge of the legal framework and statutory obligations under the Mental Health Act, Mental Capacity Act
  • Detailed understanding of Safeguarding and its application in practice
  • Clinical Risk Assessment and Management and its application in practice.
  • Clinical Governance and its application in practice.
  • Working knowledge of clinical medicines management
  • Understanding of psychological models of care and treatment (depending on specialist area)
