Responsibilities of a Mental Health Nurse Practitioner Band 6:

– This post is open to professionally registered qualified staff who are registered as a Mental Health Nurse, Occupational Therapist or Social Worker.

-The post holder will provide community-based engagement and assessment to individuals with complex mental health, substance misuse, physical health and social care needs in partnership with mental health and addiction services.

-The post holder will be working alongside Bolton Local Authority, GMMH RSDA Recovery Coordinators and assertive outreach workers providing mental health and addiction support to service users who are homeless or at risk of becoming homeless.
