The postoffers:

Deliveryof primary care within a health and social model

Fascinatingwork with a diverse patient group, with a list size of approximately 6500 overboth sites

    JOB PURPOSE: To provide primary medical care for homeless people, people in insecureaccommodation, refugees, asylum seekers and all other patients registered atBevan House Primary Care Centre (the Practice), at the practice, and tocontribute to the development of health and other services for these groups

    Roles and Responsibilities

    Clinical

    Deal promptly with patients problems,providing usual primary care based treatment and referring as appropriate

    Offer full preventive health careservices

    Work closely and co-operatively withother members of the practice team and partner organisations, providing adviceand support when required, particularly in the area of substance misuse.

    Work to ensure that the practiceachieves its quality outcomes and contractual targets

    Keep careful medical records, andcollect data required for health needs assessment, and for monitoring practiceperformance

    To actas an advocate for patients when appropriate.

    To ensure data protection guidelines arefollowed, confidentiality is respected and patient access to services ispromoted

    Be involved in audit, service reviewand if appropriate research.
