Job summary

You will be a part of a team of Young Peoples Housing Support Mentors working under the direction of the service manager to deliver support to the clients, providing high quality and effective housing related support to those aged 16-25 within the 24-hour supported housing property.

Support will focus on young people’s strengths and goals according to their individual needs, within a framework of reducing harm, achieving tenancy sustainment, and living independently.

Staff will work flexibly across the hours and days of service delivery (7 days per week, 24 hours a day) and will be part of a rota of on-call service for out of hours provision where necessary.

Main duties of the job

  • Provide a variety of person centred, tailored interventions that support individuals with complex needs to find sustainable accommodation and develop their independent living skills.
  • Manage a client caseload, and complete individual needs assessments, risk assessments, support plans and offer practical advice and advocacy to service users.
  • Support service users to identify the areas of support needed and identify the best option of support based on ongoing needs as outlined in their individual Support Plans.
  • Liaise with relevant services, organisations, and agencies with regards to accessing additional support for service users.
  • Practice homeless prevention, tenancy sustainment, Psychologically Informed Approaches, Solution Focus Practice, harm reduction, and co-production.
  • Complete needs assessments, risk assessments, and support plans for service users.

    Four charities in Wales joined together on 1st April 2021 to become Adferiad Recovery, a new organisation which will provide support for vulnerable people in Wales and their families and carers. Adferiad Recovery will have a particular focus on people with mental health problems, substance misuse problems, and those with co-occurring and complex needs.

    The merging charities are:

  • Adferiad Recovery, based in South Wales, provides services for people with co-occurring mental health and substance misuse conditions and related issues
  • CAIS, based in North Wales, provides substance misuse services, employment, mental health, and veterans services across Wales
  • Hafal, based in South Wales, supports mental health service users, carers and other vulnerable groups across Wales
  • WCADA, based in South Wales, provides substance misuse services in Swansea, Neath Port Talbot and Bridgend and the Public Prisons at Swansea, Cardiff, Usk and Prescoed.
  • CAIS, Hafal and WCADA had recently worked together to form Adferiad Recovery as a new charity. This enabled the charities to work together more closely, and the four have now merged into this new single charity.
