An exciting opportunity has arisen for a dynamic and enthusiastic Band 6 Community Nurse to join our Primary Care Team delivering health care to the Homeless community in Birmingham. We are based in a GP practice near the city centre dedicated to the Homeless and have well established links to all other agencies involved in the care of the homeless including Birmingham City Council Homeless outreach teams, Birmingham and Solihull Mental Health Trust Homeless Community Mental Health Teams and associated 3rd sector partners in the care of the Homeless.

We are a hardworking team and require someone who wants to be a team player but can work on their own initiative to develop services and is flexible enough to meet the challenges of the post. An interest in women’s health would be welcomed. The post holder must be motivated to work with a variety of agencies including hostel staff, accommodation providers and outreach workers to assertively engage with service users who may be experiencing complex and significant health issues.

Main duties of the job

This is a new post to work in a mainly outreach role, alongside our experienced homeless outreach nurse, to deliver care to our most vulnerable patients who do not actively engage with their care or the service and are often experiencing serious health issues as a result of their substance misuse and lifestyle.
