Job summary

The post holder will act as a senior clinician working with homeless and vulnerably housed adults with complex needs within Abertawe Medical Partnership. The role specifically entails providing specialist primary care based support to homeless/vulnerable patients, with a history of mental health and substance misuse. To ensure the delivery of high quality care and services to this patient group and to ensure that this is delivered in accordance with key local and national policies.

Main duties of the job

To act as care co-ordinator to the services patients-assessment of treatment and care needs.

To work with a philosophy of harm reduction.

To assertively engage patients in treatment by working in a flexible manner, including working from a variety of statutory and non-statutory settings, actively seeking to retain patients in treatment.

Develop and maintain effective liaison with other agencies working with patients including CMHTs, in-patient Mental Health and General Hospital Services and non-statutory services.

Frequently manage extremely distressing situations.

Able to provide a variety of interventions involved in the care of patients with a duel diagnosis and/or poly drug use including alcohol misuse.

Recognise a range of physical health problems associated with both drug use and homelessness.

Ability to work with patients with a history of abuse.

Deliver prepared education and training to groups of non-clinical staff on Borderline Personality Disorder.

Large MDT comprising of 6 partner practice, 2 salaried Drs, 4 part time PNs, Cluster Pharmacist, Cluster Paramedic with additional support from the Mental Health team and the Community Outreach Mental Health Homeless Nurse. Based on two sites.
