Social Worker: Haringey Homeless Health Inclusion Team (HHHIT)

Role Summary:

HaringeyGP Federation is recruiting for a Social Worker for the Haringey HomelessHealth Inclusion Team. This is a new role with our service intended to not onlybridge the gap to improve patient access to Primary Care services, but also tosupport patients across the social spectrum; enabling patients to access thesocial care that they may need to live a more independent life.

Aspart of the service, outreach to street sites and temporary accommodation willbe undertaken, and the role of the social worker will support the individualsonce first contact has been made.

Thesuccessful Social worker will be part of our Operational Team, working todeliver the key objectives of the Federation, supporting and strengthening GeneralPractice, providing at-scale Primary Care services and being a voice of PrimaryCare in multi-agency working. This is a uniquely rewarding opportunity for a motivated,organised and enthusiastic individual who would like to gain experience withina young and innovative team. Due to the nature of the role, this may be officebased as well as visiting patients across Haringey.

Hours of work: Full Time -37.5 Hours

Remuneration: £42,000.00to £45,000.00 per annum

Contract length: Fixedterm contract for 2 years

Main Dutiesand Responsibilities:

• Toidentify and challenge barriers to reducing existing street homelessness andaccessing support and to advocate for access to assessment and services.
• Toundertake assessment and support planning in a person-centred and holisticmanner which focuses on both the individuals strengths and preventativeoptions that enable us to enhance individual independence.
• Towork in partnership with the Rough Sleeping Taskforce to contribute to thereduction of rough sleeping in the borough of Haringey.
• Toparticipate actively in case conferences related to rough sleeping individuals whererequired and participating in other meetings or working groups relating torough sleeping.
• Tomanage and prioritise workload so that deadlines are adhered to with a focus onhigh standards of written work and effective interventions / outcomes.
• Toassess, review and providesupport to carers / family where appropriate
• To ensure that all assessments are in linewith relevant adult social care legislation and to adapt working style to meetthe needs of those who are rough sleeping; recognising where individuals mayhave difficulty engaging in the process and adapting accordingly.
• To carry out mental capacity, Care Act 2014 andno recourse to public funds assessments and make best-Interest decisions whereappropriate.
• Keepfull, clear and accurate records.
• Toensure that any changes to support arrangements are actioned and that cases arepresented to panel where additional funding is required. Where reductions inpackages are appropriate Social Worker to work with brokerage to make the requiredchanges. Social Worker will also be responsible for pulling back unspentpersonal budgets and to ensure that care packages are value-for money.
• Towork within the framework and principles of the Care Act 2014 in making surethat the assessments / reviews are accessible, person centred, holistic with afocused on wellbeing and prevention.
• Beprepared and fully participate in supervision.
• Tounderstand and work with the Continuing Health Care framework, including thecompletion of DSTs and joint working with health colleagues.
• Workclosely with commissioning to ensure that quality standards of providers arebeing met and to raise concerns or issues when they arise.
• Toensure the safety of service users by working in collaboration with partners,providers and carers to ensure that risks are managed and that positive risktaking is supported.
• Tounderstand the Haringey Safeguarding Process and the safeguarding statutoryguidance as set out in the Care Act 2014 and ensure that any identifiedsafeguarding concerns are raised appropriately and in a timely manner, with afocus on Making Safeguarding Personal.
• Toundertake safeguarding enquiries.
• Understandand work to the legislative and organisational frameworks to ensure dischargeof statutory duties while maintaining best practice standards through allcontacts.
• To actively participate in team meetings to look at new ways of working,improving outcomes and best practice standards to help to the development ofthe service.
• Work positively and proactively with a range of professionals, organisations and carers to make sure targets are set, outcomes are achieved and life opportunities maximised for those that we support.
• Be clear on what the performance management indicators within the Assessment Service are and ensure that you are able to meet these targets.
• Have a clear understanding of the resources, services and provisions within the borough; particularly within the voluntary and community sectors, to ensure service users and their families are connected to these opportunities.
• To exercise responsibilities under the Health and Safety at Work Act, 1974, as laid down, or as may be amended by departmental procedures from time to time.
• To continue professional development by undertaking mandatory training and other training identified in line with identified needs
