Job responsibilities

  • Provide a holistic assessment of mental health care needs for patients referred to the service whilst also promoting the uptake of health screening and preventative health care where indicated.
  • Develop, implement and evaluate episodes of care to meet immediate needs, including onward referral to primary health care teams, other health providers, disciplines and third sector agencies.
  • Therapeutically engage with individual Services Users who may have complex health conditions to support them with their short-term mental health needs.
  • Work alongside the lead health or social care team to offer specialist assessment, advice/treatment and pathway of care for patients mental health and wellbeing support.
  • Provide expert support and advice to colleagues within the healthcare teams and local authority in order to enhance engagement, rehabilitation outcomes and service success when working with individuals presenting with physical health needs alongside mental health needs.
  • Use specialist knowledge to assess and triage the level of clinical input for new referrals if mental health issues are indicated, and act as a liaison to AWP/Social Services/IAPT as necessary
  • Contribute to staff development and learning by providing in house training on issues as requested by the organisation and teams.
  • Liaise with specialised services (e.g. counselling, AWP, sexual health services, drug and alcohol liaison) in order to improve working relationships and to advocate on behalf of patients
  • Be able to effectively communicate and in a timely manner at all levels within Primary and Secondary Care, to a variety of health professionals, patients and their carers to provide the best outcomes for the person with mental health problems. This role will require the ability to interface with a range of professionals and agencies that may face different constraints on a day-to-day basis
  • Contribute to clinic sessions as required to develop networking with other specialist services ie Homeless Health, The Haven, TB team
  • To participate as a team member of SASS Community Mental Health Nurse Team in shaping and developing the service
  • Participate in conducting audits of quality care and clinical effectiveness, to include risk, NICE Guidelines and CQC requirements
  • Ensure compliance with policies, procedures and clinical guidelines for self and others and participate in the development of the same
  • To attend necessary meetings as part of service delivery and the review and evaluation of the same
  • Work flexibly to meet the service needs and undertake any other duties that may be reasonably requested to help the service run smoothly
  • Be able and willing to travel throughout the service delivery geographical area

  • Evidence of relevant continuing professional development, evidence of post registration qualification in community health or willingness to undertake.

  • An understanding of and interest in community MH nursing, and its impact on individual and public health including the implications for service delivery of cultural and social diversity and inequalities issues
  • Knowledge of commissioner provider issues and how these impact on service provision and an understanding of key national and local priorities for community MH nursing
  • Expert assessment and case management skills to support patients and teams.
  • Providing individualised patient care based on the principles of evidence-based practice. Acting as patients advocate as required
  • Expert skills to communicate effectively with the team, client groups and patients from a wide range of social and cultural backgrounds, including those who are socially excluded.
