Llwchwr LCC is seeking someone who enjoys working both within a team and independently, who can work closely with both Clinical leads and the Health Board in a busy fast paced environment.

Main duties of the job

Llwchwr Local Cluster Collaboratives (LCC) is one of 8 LCC networks in Swansea Bay University Health Board. Geographically covering: Gorseinon, Loughor, Penllergaer, Pontlliw, Pontardulais, Gowerton and Penclawdd. Llwchwr LCC has geographical challenges and different patient populations with key health issues including obesity, low level mental health challenges in addition to an ageing population. The LCC provides the opportunity for the implementation of innovative approaches; a testing ground for improving health and wellbeing in an area with complex health and social care needs. Working to:

Prevent ill health enabling people to keep themselves well and independent .

Develop range and quality of services that are provided by the wider primary care in the community, including dental, optometry, and pharmacy.

Ensure services provided by the wide range of health and social care professionals in the community are better coordinated to local needs.

Improve communication and information sharing between different health, social care and voluntary sector professionals.

Facilitate closer working with community based and hospital services, ensuring that patients receive a smooth and safe transition from hospital services to community based services and vice versa.

Develop links with partners such as Social Housing, Health of Homeless and Vulnerable Groups Forum, Carers Associations, South Wales Police, Fire Service and Rescue Services, and Schools and Colleges
