Job summary

This role is an exciting outreach role across the whole of the County of Durham offering support to the prescribing team and Humankind teams to engage hard to reach service users that have continued heroin use or have not been in service previously. We have a cohort of homeless SU with complex needs and conditions and we see this post as taking services to the service user to promote engagement with normal established services. The role will work along side the MH nurse role and will support services both in person and via teams to enable MDT approach within other services. support Humankind RC with complex cases and complex needs, promoting service to our stakeholders and partners

Main duties of the job

Engaging with hard to reach and complex service users, manage a small caseload for a specific time limit before accessing main service engage recovery workers with complex needs. Liaise with key stakeholders to enhance service delivery work with Mental Health service and housing services, work collaboratively with MH nurse role to promote service engagement. Promote CDDAS with all other service and contribute to KPI compliance plan. NMP course engagement to enhance the delivery of this role. Access additional training around wound care, tissue viability source course and present case for additional training develop Doppler training and use.

  • RNA/RNMH/RNLD
  • Educated to diploma level or above

Desirable

  • Specific misuse qualification (certificate in addiction studies, certificate in management of substance misuse)
  • Qualified Independent Prescriber or in a position to train within a year of commencing the contract.

Knowledge and Skills
