Outreach Mental Health Nurse Job Vacancy in Hounslow, England, UK

HCL Mental Health are seeking a highly skilled and compassionate Outreach Mental Health Nurse (RMN) to join our team in Hounslow. This is a 12-week vacancy requiring 37.5 hours per week. There is potential for this role to be extended.

As an Outreach Mental Health Nurse, you will be responsible for providing specialist drug and alcohol outreach clinics for local hostels, homeless individuals, and other services in the Borough of Hounslow. Working as a member of a multidisciplinary team, you will provide a range of treatments and services to problem drinkers and problematic drug users, including harm minimisation approaches, motivational interviewing, relapse prevention, and ITEP care planning.

Responsibilities:

• Conducting comprehensive mental health assessments to identify client’s needs and determine appropriate interventions or referrals to other mental health services
• Promoting health and well being through a range of health checks, including ECG, blood borne viruses screening, vaccination, and Fibro scan, to improve client’s overall physical and mental health
• Assessing and case managing clients who inject drugs, including assessing injection sites and utilising harm minimisation principles to reduce potential harm and negative consequences of drug use
• Having a professional duty of care to protect clients who present a risk to themselves or others, including implementing risk management strategies and following protocols for reporting any concerns
• Complying with statutory requirements regarding safeguarding children and child protection, including the Children Act 1989 and Hidden Harm guidelines, to ensure the safety and well-being of all clients, particularly vulnerable children and families.
