Were currently recruiting into our Rough Sleeper provision,targeting homeless individuals, offering a flexible and outreach response tohighly vulnerable individuals sleeping rough or living in temporaryaccommodation.
As an AdvancedPractitioner we offer a starting salary of £26,000 rising each year in linewith our pay progression salary bands, rising to £30,416 per year. (Dependenton experience you may be offered a starting salary above the initial startingsalary).

Main duties of the job
Carry adesignated caseload of service users with complex needs that may include one ofthe following: mental health, long term conditions, criminal justice, offendingbehaviour, homelessness, motivational interviewing, safeguarding, family work;
Conductassessments of need and risk assessments and formulate risk management plansand recovery plans that recognise the personal recovery goals of the serviceuser;
Effectivepartnership working with members of any multi-disciplinary team and otheragencies and coordinate multi-agency meetings when appropriate;
Deliverinterventions that are underpinned by a strong evidence base;
Participatein clinical meetings and complex case meeting and be confident and competent atpresenting cases orally and through written reports;
Supportthe learning and development of Recovery Workers and Trainee Recovery Workers,providing specialist input into Multi-Disciplinary Meetings;
Providesupervision, mentoring and coaching to staff to support their development,learning and standards of care;
Beingknown as a knowledgeable in a designated specialism within the service use and mental health, Homelessness/complex needs, domestic abuse,mental capacity);
Ensuringachievement of key performance indicators relevant toyour role.
