We are looking to employ an Associate Practitioner to join our developing Flow Management Team in the Care Trust

Potential candidates will be enthusiastic individuals who have previous experience as a Band 3 or Health Care Assistant working within an inpatient or community setting; who can deliver recovery focused nursing interventions, provide housing and social care related support, and are keen to further develop their knowledge and skills.

The post holder will work across both Inpatient sites of the Trust and be part of the Flow Management Team. They will work in a supportive environment that is committed to personal and professional development through regular clinical / managerial supervision and ongoing training.

A flexible and solution focused approach is required as this post involves the need to work around the rapidly changing demands of our service users.

Main duties of the job

  1. Job Purpose:
  • The main purpose of this job is to support timely discharge and transfer of care of service users in the Care Trust and associate services / bed providers. This will be done by providing support for service users to find appropriate, suitable, and safe accommodation and benefits advice, alongside other factors that may influence delays in movement
  • The post holder will undertake assessments with the patient and relevant professionals to ensure suitable accommodation is found, completing the ‘Duty to Refer’ under the Homelessness Reduction Act (2017), supporting with housing register applications, benefits enquiries (for those on Job Seekers Allowance and those on Employment Support Allowance (ESA)
  • Provide support as directed by the Flow Manager in the administration and completion of required documentation to assist the Bed Manage
  • Support Care Act Assessment completion via direct communication with Care Co-ordinators and the MDT
  • The post holder will be a point of contact for housing related referrals where the patient cannot return home or is homeless. This work will be carried out in collaboration with the patient, MDT, Local Authority, and all necessary and appropriate community agencies
