Job summary

The Changing Futures project is at the forefront of innovative, person-centred, multi-disciplinary working, striving to provide a pioneering multi-disciplinary assessment and support service. The team worksholistically with people in the city of Brighton & Hove who have multiple and compound need including mental ill health, homelessness and eligible care needs arising from substance use. This project has been given approved funding until October 2024.

Main duties of the job

This service adopts the NICE clinical guidelines around integrated health and care for people experiencing homelessness to ensure best practice is being followed. The service works within the framework of wider ongoing work around system change, needs mapping for the Multiple Compound Need cohort, and assessment of future commissioning need across health, housing and social care.

As the Clinical Nurse Specialist, you will be working alongside colleagues in homelessness prevention, mental health, psychology and social work, building a broad range of experience. We believe that, working together we can make a real difference to people in this truly integrated model of service. The newly developed multi disciplinary team sits in a wider nexus of existing statutory and voluntary sector services to ensure that people’s full range of needs including physical health are met and people are supported to retain or move into suitable accommodation.
