Job summary

Solent GP Surgery care for a total of 20,000 patients within Southampton City with sites at Royal South Hants Hospital, Portswood and Adelaide Health Centre. We work alongside our Homeless Healthcare Team who currently have 400 patients registered with them.

We have an established medicines team consisting of Clinical Pharmacists, a Primary Care Network Pharmacist, Medicines Manager and prescription clerks.

We’d really welcome a Clinical Pharmacist who is passionate about putting patient’s first, loves working as part of a multidisciplinary team and really values learning and development. The successful candidate will benefit from support and supervision from a GP Prescribing Lead, other Pharmacists within PCN, the medicines management team at the CCG and a Trust Pharmacy team with a Chief Pharmacist

The Clinical Pharmacist will provide remote or face to face medication reviews, review of patients with polypharmacy and multiple comorbidities and to assist in the management of patients with long term conditions, medicines on transfer of care and systems for safer prescribing, repeat prescription authorisation, and reauthorisation, as well as acute prescription requests.

The successful candidate will provide leadership on quality improvement and clinical audit, learn to co-ordinate some aspects of the Quality Outcome Framework (QOF)/ Primary Care Network (PCN) specifications and manage long-term conditions. Ideally, we would like to recruit someone who is already a non-medical prescriber.

Main duties of the job

Review patient requests for repeat prescriptions and review medicines reaching their review dates; issue prescriptions and make necessary changes as an independent prescriber, and ensure patients are booked in for necessary monitoring tests where required.

Undertake clinical medication reviews with patients with multi-morbidity and poly-pharmacy and implement own prescribing changes (as an independent prescriber) and order relevant monitoring tests.

Implementing medication changes detailed on discharge summaries. Liaising with Consultants and GPs as appropriate

Making changes to medication following updated guidance or safety alerts being received and advising both patients and clinical staff on the change.

Actively participate in clinical audit through data collection to ensure policies and guidelines are adhered to.

Independently assesses patients for their eligibility for a medicines compliance device from a community pharmacy using professional judgement and conducting a medication review.

Play an active part in the development and implementation of evidence based policies and guidelines. Evidence based guidance development maximises the benefit of medicines to patients, controls cost and minimises risk.

Independently and proactively provide support and advice to GPs on the analysis and monitoring of prescribing trends including the interpretation of PACT data.
