Prison Resettlement Outreach Worker

We are recruiting for a Prison Resettlement Outreach worker to work for a charity based lient that is dedicated to supporting some of the most vulnerable and disadvantaged people in society through their services and projects across the South West.

The Prison Resettlement Outreach Worker will meet clients in prison, taking a person centered approach to encourage engagement with substance misuse services, mental health services, and probation. They will take an assertive approach to prevent losing contact with rough sleeper’s post-release, ensuring they have suitable accommodation for their support needs.

Main Responsibilities;

  • To undertake assertive outreach and hot spot visits across the locality area including during unsocial hours, maintaining a regular street presence
  • To track the progress of the referrals made in respect of identified rough sleepers
  • To build relationships with people who are rough sleeping, using inter-agency outreach sessions and regular visits to agencies/venues visited by people who are rough sleeping
  • To give clients advice, information and provide an enabling service so that they can access appropriate services
  • To carry out risk assessments and complete Housing Benefit forms for rough sleepers moving into designated accommodation services
  • To recognise indications of substance misuse, mental health needs, personality disorder behavioural issues and make appropriate referrals to relevant agencies
  • To promote and facilitate effective multi-agency and joint working to address begging, street drinking and other street-based activity
  • The focus of this role is to support rough sleepers who are imprisoned, by staying in contact during their sentence, identifying and arranging appropriate accommodation in advance of their release date. As a result, this post will see a reduction of homeless reoffending, and rough sleepers returning to prison.
  • The successful post holder will work closely with substance misuse services, mental health services, Probation, HMP Bristol and HMP Eastwood Park, and Avon and Somerset Police.
  • The Prison Resettlement Outreach Worker will work as an integral part of the Outreach team, contacting new, repeat and entrenched rough sleepers in Bath, implementing a locally agreed approach to No Second Night Out.
