At UHBW we are creating two new Transfer of Care Hubs to support our patients with discharge planning from admission through leaving hospital and into the community; one Hub at the Bristol Site and one at Weston General Hospital. The Transfer of Care Hub will bring together our community partners (Sirona), Local Authorities and Voluntary Sectorcolleagues across BNSSG tocreate a new team from multidisciplinary backgrounds.

The Transfer of Care Hub will bring together, Acute Trust Case Managers, and Patient Flow Coordinators, Social Workers, Local Authority Discharge Coordinators, Sirona Case Managers, Community OT’s, the Homeless Support Team, Voluntary Services and Flow and Discharge Coordinators.

We are looking to recruit a Homeless Support Team Acute Trust Case Manager (for Bristol site with some telephone support to Weston site ) to join the Transfer of Care Hub.

You will be part of the Homeless Support Team, working with patients experiencing or at risk of homelessness supporting them through the hospital pathway. We aim to encourage and support engagement with health and social care services, improving health outcomes and ensuring safe and timely discharge into accommodation whenever possible

Main duties of the job

You would be joining a dynamic patient-centred team, providing an excellent service to our patients and ward colleagues. Working within a supportive and friendly environment, you will be encouraged to participate and share learning with colleagues across the team.

We are looking for a nurses or AHP to join our team to promote heath inclusion for our patients, improving their experience of using services within the hospital and the discharge pathway.

We are looking for an outstanding individual who:

  • Always puts the person at the centre of their care
  • Has experience , knowledge or an interest of those experiencing homelessness and severe and multiple disadvantage
  • Is enthusiastic, driven and practical
  • Has excellent communication and engagement skills working within a trauma informed approach
  • Is able to prioritise workload, problem solve and work flexibly and responsively
  • Can use their own initiative, be able to manage their own caseload as well as working as part of a team

This is a full time post hours a week with scope for flexible working with Secondment considered
