Outreach Support Worker
BS Social Care are currently recruiting for experienced Outreach Support Workers to work with Adults with Mental Health needs.

General Responsibilities include:
- Work alongside partner organisations to build relationships, and ensure all individuals understand their rights and responsibilities, having access to the right tools and resources, to support them back into society and building networks.
- Support all individuals on the caseload through one to one meetings and group settings, ensuring comprehensive assessments, risk assessments, and support plans are completed are completed to a high standard.
- Attend and contribute to blended triage and other meetings with partner organisations to support individuals to achieve their personal goals, in line with their support plan.
- Build strong and sustainable relationships with internal and external stakeholders and organisations. Some of these include but are not limited to; Homeless Prevention Officers, Local Landlords, Housing Benefit Teams, Temporary Accommodation and Housing Associations, Hostels, Rehab Facilities, and other relevant organisations.
- Be an active team player and support the team and also the service users to meet goals and targets.
- Undertake regular supervision with company’s operational management and other relevant persons to provide, when required updates on clients who have been transferred to primary care for additional support.
- Participate in training in utilising partner organisations client electronic records and any other recording system as deemed necessary for client support needs.
- Administration duties will vary. This includes maintaining confidential records in a timely manner and containing information in accordance with relevant statutory and organisational policies.
- Follow safeguarding protocols and make referrals where necessary.
- Adhere to health, safety, and privacy requirements and follow correct procedures with reporting incidents, accidents, and risks.
