Job summary

We are seeking to recruit an experienced social worker within our County Wide Hospital Discharge Service. This post is based at Chesterfield Royal Hospital Discharge Team but there will be an expectation to provide support to other hospitals when required.

Main duties of the job

The team primarily provides short -term assessment and support to facilitate safe transfers of care, or to prevent unnecessary admission to hospital and to enable people to continue living in their own home as long as possible. Post holder will be expected to work within tight discharge timescales and therefore will be of interest to applicants who enjoy working within a busy integrated Multi-disciplinary setting.

You will need to be committed to working in a person centred and strengths-based way, recognising peoples assets and strengths and build networks and positive relationships with Acute and Community Trust health colleagues and agencies.

We expect that applicant to be confident in using the Care Act and have a good understanding of Making Safeguarding Personal and the Mental Capacity Act.

The successful applicants will need to work in a team setting, cover part of a duty rota system and be expected to manage a busy, complex caseload. Good communication with other agencies is essential.

Experience of working in Adult Care, is beneficial but not essential. You will need to have transferrable skills to enable you to facilitate safe, sustainable transfers of care for people utilising a Home First approach.

  • Record all work undertaken and prepare reports/assessments.
  • Accept responsibility, with the line manager, for reviewing work on caseloads at regular intervals. This would include closure and transfer of cases.
