Job summary

The homeless hospital discharge service providesenhanced inpatient care, and ensures safe, appropriate and sustainable hospitaldischarges that meet the needs of patients currently experiencinghomelessness.

The service works with a variety of hospital,statutory and voluntary partners to achieve these objectives and aims to bringa wider influence to hospital, housing and social care system to proactivelyidentify and better respond to the needs of currently homeless and otherwisedisenfranchised people.

The overall aim of the SupportWorker post is to provide a supporting role to work within the BradfordHomeless Discharge Team. The successful candidate will be responsible for theidentification and assessment of homeless/unsuitably housed patients admittedto Bradford Teaching Hospitals alongside other team members. This will includeattendance at multi-disciplinary meetings and the need to provide communitysupport to patients discharged from hospital.

They will be expected to workclosely with the Lead Nurse and Homeless Discharge Team. The team works closelywith staff across all disciplines between hospital and community to ensure asafe, timely and effective transfer of patient care on discharge.

Communication and team workingis an essential aspect of this role. The successful candidate must be able towork both autonomously and well within a small team.

Main duties of the job

Carryout a holistic assessment in order to address both physical and social needs,subsequently assisting in the setting up of support packages based on theirassessment. This will include support with the patients housing, physicalhealth needs, alcohol and drug dependencies, mental health issues and any othersocial care needs that the patient would benefit from been supported with.

Develop and maintain links, working in partnershipwith external services such as local authority housing departments, supportedaccommodation providers, street outreach teams, local specialist homeless GPservices, substance misuse, and other relevant services to ensure patientscare plans are coordinated effectively.
