Job summary

The service will deliver asylum seeker health assessments and infectious disease screening to asylum seekers based at TLK Apartments, 36-40 High Street St Mary Cray, BR5 3NJ.

The service will be delivered by an Advanced Nurse Practitioner working closely with the registering practice and will be delivered on behalf of the registering practice.

This model of care is similar to that commissioned from BGPA for the Homeless Health Clinic.

There are 68 residents, adults and children resident at the site. All are to be offered health assessments.

Pay range of £ to £ per hour depending on experience.

Main duties of the job

Engage with the residents and build trust.

Adopt a trauma informed approach.

Arrange consultations with residents in a location that is appropriate with respect to confidentiality and safety. This may be at the registering practice if most appropriate.

Arrange for professional translation services to be scheduled to support when necessary (as commissioned for the registering practice)

Liaise as necessary with the lead GP at the registering practice.

Attend the Asylum Seekers Health Group meetings (one hour every 4 to 6 weeks approximately)

Make onward referrals as necessary and appropriate to safeguarding, mental health, chest clinic using established pathways.

Facilitate the arrangements for immunisations and screening to be scheduled by the registering practice.

Arrange for blood tests to be carried out using the registering practices usual arrangements, or where appropriate carry out the blood tests.

Ensure IGRA test samples appropriately managed.

Cover content as described below in the health assessment and screening.

Record clinical findings and actions on the EMIS system of the registering practice.
