An exciting opportunity has arisen within Active Programmes team for a Band 6 Specialist Active Programmes Nurse delivering rehabilitation to both pulmonary & cardiac patients. The team is based in the community and the vacancy would be expected to work city wide. The team also has a Mental Health team who support patients manage their respiratory and cardiac health.
The variety of conditions seen makes this post ideal for developing your knowledge and skills in this key area of nursing which is increasingly in focus due to the current COVID 19 pandemic and rehabilitation needs for this group of patients.Â
Main duties of the job
To assess, diagnose, plan and develop specialised rehabilitation programmes for patients referred to the Active Programmes team.
To supervise/train inexperienced nurses/assistants/students working within the service.
There is a well developed multi disciplinary team of nurses, physiotherapists and specialist Therapy assistants all of whom cover both the Pulmonary and Cardiac rehabilitation. There is a Clinical Nurse Team Leader, to support you in developing your knowledge and skills in this clinical area and you will work closely with Specialist Physiotherapists within these areas and so should be used to working in a multidisciplinary environment.
The successful applicant will be a registered general nurse, have evidence of post graduate training in pulmonary and/or cardiac conditions, have excellent communication skills, time management skills and have the ability to work on their own initiative. Evidence of community working experience pre or post registration is desirable.
